JHSC and LEC Member Questionnaire

December 2009
SURVEY QUESTIONS FOR WORKER JOINT HEALTH AND SAFETY COMMITTEE MEMBERS AND HIGHEST RANKING BARGAINING UNIT/LEC MEMBERS IN LONG-TERM CARE SECTOR

This survey is to gather information about how Joint Health and Safety Committees (JHSC)’s work in the long term care sector and to find out how connected the JHSCs are to local union structures.  The survey has been created jointly by OPSEU and our partner in the project, Occupational Health Clinics for Ontario Workers (OHCOW). OHCOW’s interdisciplinary team of occupational nurses, physicians, ergonomists, and hygienists bring current research and experience into workplaces to help prevent injuries and occupational disease.  OHCOW will use their expertise to assist us in identifying and developing tools for OPSEU LTC members to work as effectively as possible at their JHSCs which will ultimately improve health and safety in their workplaces.
OPSEU is asking that:

  • the highest ranking LEC member in each bargaining unit fill out the survey and,

  • identify, approach, and request that all worker joint health and safety committee members of their JHSCs complete the survey before Jan 29, 2010.

If you have already sent in a completed paper survey, you do not need to complete it online.
If you have any questions about the survey and how to complete it, or who completes it, please contact Terri Aversa, OPSEU Health & Safety Officer, at OPSEU: 416-443-8888 (or toll free 1-800-268-7376) ext 8774 or taversa@opseu.org.The survey will be open until Feb 1, 2010
