Thank you for taking time to complete the 2017 Inclusive Workplace Culture survey. Your insights strengthen Mosaic’s advocacy of the business case for diversity and inclusion in the Tulsa region.
The survey process includes two separate questionnaires: one targeted to organizations with 1-50 employees (as aligned with the Chamber’s Small Business Connection membership), and one for organizations with 51 employees or more. This segmented approach allows Mosaic to better engage with and accommodate the unique position of small businesses and sole proprietorships.

To receive Mosaic’s Top Inclusive Culture recognition, respondents will need to meet a minimum threshold for each of our five key pillars: CEO Commitment, Diverse People, Internal Policy, Community Outreach and Diverse Suppliers.
As you progress through the questions, feel free to save your work and finish at a later date. If you choose to 'save and complete later' you will be given a link to return to your survey. You will have to 'click thru' your survey to get to the point at which you stopped. Also, you may use your browser's 'back' button to go backwards in the survey and change any of your answers.
Incomplete surveys will not be counted or reviewed for the Inclusive Workplace Cultures recognition. Survey responses are due by the close of business Monday, August 7.
